The charitable funding we receive from the Masons of California allows the Masonic Center for Youth and Families to offer accessible treatment and flexible payment options. Each potential client is reviewed on a case-by-case basis to ensure that no one will be denied services due to a lack of resources.
When possible, we may bill your private health insurance to receive payment for services provided. In such cases, we may provide protected health information to the insurer, including information that identifies the client, their diagnosis, and the types of services provided by MCYAF.
In the event there is a disruption to your insurance and we are unable to bill your insurance provider, clients will be responsible for the payment of services to MCYAF.
We can also provide our clients with the documentation necessary to submit claims to their own insurance plan. The clinician who works with each family will assist in reviewing the range of recommended services and establishing a payment plan that fits each family’s budget and needs.
During the course of providing services to you, Masonic Homes of California doing business as Masonic Center for Youth and Families (MCYAF) gathers, creates, and retains certain personal information about you that identifies who you are and relates to your past, present, or future physical or mental condition, the provision of mental health services to you, and payment for your services. This personal information is characterized as your “protected health information.”
This Notice of Privacy Practices describes how MCYAF maintains the confidentiality of your protected health information, and informs you about the possible uses and disclosures of such information. It also informs you about your rights with respect to your protected health information.
All COVID-19 related policies follow applicable recommendations issued by the Center for Disease Control and Prevention, the San Francisco Department of Public Health, and other government agencies. They have been reviewed by Masonic Homes counsel, and are available upon request. Please email information@mcyaf.org.
